Biomarkers for the detection of progressive early Alzheimer’s Disease

Background We investigated the ability of MRI, plasma, and cognitive biomarkers in detecting preclinical or prodromal Alzheimer’s Disease (AD) who will progress to next syndrome stage continuum. Method 589 subjects with longitudinal data were recruited from Neuroimaging Initiative (ADNI), which included 227 unimpaired (CU) 362 mild impairment (MCI) subjects. Preclinical AD was defined by a low amyloid beta (Aβ)42 (A+) high phosphorylated -tau (p-tau) (T+) on cerebrospinal fluid (CSF) assessment at baseline. The following baseline MRI derived an automatic segmentation tool (AccuBrain®): AD-resemblance atrophy index (AD-RAI), quantitative medial temporal lobe (QMTA) hippocampus volume (HV). Plasma p-tau 181, neurofilament (NFL) APOE ε4. Montreal Cognitive Assessment (MoCA) score collected for all Conversion (C+) as progressed CU MCI dementia within 4 years. Results Of (mean [SD] age, 72.2 [6.9] years; 314 men [53.3%]), 96 (16.3%) A+T+C+ 180 (30.6%) A+T+C-. In ROC analysis, AD-RAI achieved best detection than other individual biomarker sensitivity 83.4%, specificity 69.4% AUC 87.8%. A combination AD-RAI, plasma ε4 MoCA 89.8%, 85.1% 80.2%. subgroup also showed good accuracy identifying group (AUC = 87.6%) 89.3%). Conclusion panel might help detect progressive early
